Health Screening

The Health Screening is a short biometric examination. During the screening, your height, weight, waist, hips, blood pressure, blood cholesterol, blood glucose and HbA1c (pre-diabetic test) levels are measured by trained clinicians. We ask that you fast from food for 8-12 hours (water and black coffee are permissible).

To schedule your Health Screening appointment, you must first complete the Health Assessment. After your assessment, you'll be able to register online for your screening through your Wellness Portal. If you are a new employee, or live out of the Central Florida area, please contact Employee Choice Customer Service at 1-800-741-4846.
