Q&A About Quarter 1

What is a Health Assessment

A 10-15 minute online questionnaire used to assess your overall wellness.

When/Where will Health Screenings take place?

Altamonte Springs CREATION Health Conference Rm:
January 16-20, 2012, 7 a.m.-10:30 a.m.

Lake Mary 2nd Floor Conference Room:
January 9-13, 2012, 7 a.m.-10:30 a.m.

Out of Area employees will receive health screenings at a local AHS hospital—more specific instructions will follow

New Hires employees will receive a letter from FHHS with instructions on where to have your screening. You may also contact HR for more details.

What is measured at the Health Screening?

Your height, weight, waist, hips, blood pressure, blood cholesterol, blood glucose and HbA1c (pre-diabetic test) are measured by trained clinicians.

Do I need to do anything before I come to the health screening?

Yes. We ask you to fast from food for 8-12 hours before your appointment (water and black coffee is permissible).

What happens if I "fail" the health screening?

There is no such thing as failing. The goal is to help you improve your health.

Can my spouse participate?

Yes, in 2012 we are making certain aspects of the wellness program available to your spouse. They may participate in the health assessment, health screening and have one coach call. Their participation is completely voluntary and will not affect your quarterly requirements.

Q&A About Quarter 2

What kind of goals do I have to set?

Employees categorized as "healthy" will set one goal based on either CREATION Health or self leadership. Employees categorized as "moderate" or "high risk" will set two goals: one based on CREATION Health or self leadership and the second focused on improving health risk factors. Self leadership goals are designed to support you in developing a greater sense of well-being, building vitality and leading a life consistent with the values of CREATION Health.

If I don’t meet my biometric or lab value goal, will I still receive my incentive?

Yes. We encourage you to strive for better health. To us, the effort is just as important as the end result.

What are the self leadership online modules?

Online resources that provide information, tools and activities based on the latest research. Modules are designed to help participants through the process of purposefully engaging in change, making thoughtful decisions, having resilience and building on strengths. The modules are interactive and can be done in one sitting or at your leisure throughout the quarter.

Q&A About Quarter 3

Why is my incentive so much higher for Quarter 3 than in Quarter 2?

This year, Wellness Our Way is focusing more on coaching and making sure you connect with your PCP. Because those requirements fall in Quarter 3, we have increased that quarter’s incentive.

What are my preventative screening requirements?

Required screenings are age and gender specific:

  • Colorectal Cancer tests: men and women starting at age 50 every 10 years.
  • Mammograms: women starting at age 40 every 1 to 2 years.
  • Pap Smear: women starting at age 21 every 1 to 3 years.

Where can I find a Primary Care Physician (PCP)?

If you are a health plan member, you may visit www.fhhsonline.com to view a list of physicians within our network.

What does my PCP visit entail?

Your PCP visit is a routine annual exam. During this visit, we encourage you to speak with your doctor about getting a Vitamin D test to find out your levels and how it may improve your health.

Q&A About Quarter 4

Do I have to attend the two seminars during Quarter 4?

No. You can attend seminars throughout the year. You must complete two seminars by the end of Quarter 4. If you cannot attend in person, you may listen to the audio recording available at www.WellnessOurWay.org. This counts as credit.

Where will these seminars take place?

Quarterly seminars will be held in Lake Mary and Altamonte Springs and are available online for download for out of area employees or for those who couldn’t attend in person.
