The Health Assessment is a short online questionnaire. This is a tool that helps us assess your current health status, as well as identify ways to improve your overall wellness.

This assessment is completely confidential; your answers and results will not be shared with your employer and will not affect your health insurance premiums or amounts for co-payments.
